Super 6 Soccer has public liability insurance and professional indemnity insurance to the value of $20 million.
All our registered players and officials are covered by our comprehensive injury insurance.
If you need to make a claim please download the claim form below and fill in as much detail as possible, get your doctor to fill in his/her section, then hand it in to canteen or post it to Super 6 Soccer 194 Smith St Summer Hill.
We will fill in the remainder of the document and email it to insurance agency- Australian Sports Underwriters- Somebody will contact you to discuss your claim and give you a claim number.
Make sure you keep all your medical receipts-you will need them for claiming.
